You can find the 2026 health assessment survey online at cloverhealth.com/you or in the My Clover member portal. You can complete the survey online, download and mail a printable version, or call Member Services at 1-800-607-2348* to answer the questions over the phone. You also received a paper copy in your Clover Member Welcome Kit.

You can earn rewards by:

  • Attending a Clover-sponsored member event.
  • Taking a quarterly member poll.
  • Attending a One Pass® gym or class (in-person or virtually).
  • Registering, logging in, and navigating to the LiveHealthy app (this reward can only be redeemed once).

To schedule a LiveHealthy Visit, call Member Services at 1-800-607-2348 (TTY 711). This number is on the back of your LiveHealthy Flex Plus Visa card.

LiveHealthy rewards are subject to the following limitations:

  • They are not redeemable for cash or usable as a debit card for cash.
  • They cannot be used to purchase alcohol, tobacco, or firearms.
  • They cannot be used for pre-pay/deposit payments (e.g., hotel reservations), "drinking establishments" (e.g., bars, clubs), gambling activities (e.g., casinos, lottery tickets), legal fees/fines (e.g., alimony, child support, parking tickets), or cost-sharing and premiums.

Funds for completing the "Getting to Know You" survey, "Get Active," and Preventive Care rewards should be available on your Visa card within 72 hours of us confirming your completion. The One Pass® reward can take up to 60 days, and the LiveHealthy Visit reward can take up to 90 days.
